An individual must be at least 21 years of age, have a developmental disability and meet the medical, behavioral and staffing admission criteria required for the training program.
The enrollment process begins with:
- A referral is submitted by one of the State’s case management agencies for individuals who are in need of day programming.
- A service provider, such as Esperanza Community Services, assesses the individual, including their need for services and programming.
- Based on the assessment, Coleridge staff determines if the needs of the individual can be met at our program.
- The participant and their family/guardian meet with the program director to receive an overview of the services provided and a tour of the facility.
- Upon admission to the program, each individual is assigned to a Qualified Mental Retardation Professional (QMRP).
