Adult individuals who have a developmental disability and live at home in Chicago are encouraged to contact the Client and Family Support Program for referrals. Individuals with Home Based Support Services are accepted according to urgency for services and program capacity. Before proceeding with any services, CFSP must obtain informed consent to provide services from the guardian, adult without a guardian, or parents for those individuals less than eighteen years of age.
